End of the Semester Course Evaluation

After every semester, I love to poll my students about what they liked/disliked, what worked/didn't work in the course. I also like for them to rate themselves based on behavior, academic effort, prior knowledge, preparedness, etc. I've added sections for course content, assignments, procedures, etc. This evaluation shifts the focus onto the student, rather than the teacher. These forms push students to be reflective AND I get valuable feedback on how I can better prepare future students to take the big scary final exam. :)

I use this in my ELA course, but it can be modified to fit ANY course!

I've included my own personal version, complete with the works I use in my class.
However, I've also included a blank version for you to print and fill in. I also added a blank PPT. version that is fully editable. Finally, I added links to all clip art + fonts that I used in my version.
